Every year in preparation for our summer trips, we host one day for parents and child to come TOGETHER to the 2nd Floor of the Quigley to fill out/turn in all paperwork. This ensures we have all proper documentation for your child. We appreciate your presence on this day, it makes everything go more smoothly and safely for all.

Come to Forms Day anytime between these hours. Parent and Child MUST attend Forms Day together. It will take about 20 minutes.

2 - Insurance ID sheet (filled out or the documents to fill them out)
3 - $195 (checks payable to Holy Trinity - in memo line Urban Immersion)

Please review the Code of Conduct (click here for preview), but please DO NOT PRINT OR SIGN BEFORE FORMS DAY. We will provide the Code of Conduct onsite at Forms Day and you will sign it there.

2) Provide Medical Info for steubenville
Follow this link CLICK HERE and select 'Youth Registration' and group name 'Holy Trinity Catholic Church Lenexa'. This is medical information Steubenville Conferences require. *This is best done on a laptop or desktop.
3) What to Bring to Forms Day
Print and complete the attached documents and bring with you to Forms Day.

3 - $399 (checks payable to Holy Trinity - in memo line Steubenville)
4 - Child's birth certificate number and date of issue
5 - Your child/parent - BOTH must be present at the SAME time
6 - Parent's drivers license
4) What to Expect on Forms Day
1 - Turn in and have Youth office check all paperwork/payment
2 - Sign Code of Conduct
3 - Fill out Notarized Document (we will have notary on hand, this is proper documentation giving HT youth ministry permission to take your child across the border into Canada)
4 - Choose a bus partner for the trip
5 - Ask any questions you may have

Code of Conduct + Notarized Travel Document.

Final payment for both trips is due on Forms Day!
